Trainer Feedback Forms

Hi I am looking for any help on Trainer Feedback forms that would form part of our CIP process.
We currently have a number of approved trainers who deliver on our behalf, I want to put in a proces that captures their feedback. I already have forms to capture delegate feedback, what I am looking for is feedback from trainers. I wondered of anyone can offer any forms that they already have?
