Exit Interviews

I am currently studying the Pstgraduate Diploma in Human Resources Management. My work based project is going to be based on Exit Interviews. What procedures do you have in place for conducting exit interviews and after the interview has taken place i.e. who carries them out, do you action any recommendations from former employees, are any complaints followed up, etc?

Any help would be great.
