Employee satisfaction survey

I’ve been asked to design and carry out an employee satisfaction survey covering as a minimum communication, pay and benefits, development, etc, and inviting suggestions/ideas for improvements.
Has anyone conducted a similar survey recently who would be willing to share with me how they went about it, and any of the potential pitfalls?
