In-House 360 Degree Appraisals

I have been asked to look at setting up a 360 Degree Appraisal scheme for my company.

Has anyone had to do this in the past?

If so, are there any pitfalls that i should look out for? Can anyone suggest some guideline to work to? Or examples that i could 'borrow'?
