360 Degree Appraisals

We've been told by IIP that we need to implement a 360 degree appraisal scheme. However rather than appraisal individuals on the management team we would like to appraise the management team as a whole.

Is there anyone that has some examples of these types of questionnaires without having to pay huge fees to HR Consultants?
