A member of our team has had 6 individual days sickness in the last 4 months and has self-certified himself with the reason being migraines.
We are aware that they have been to a doctor about this.
We are concerned that these are stress related, and would like to discuss this with the employee. We would like him to investigate the causes of the migraines (whether it is stress, diet related etc) and would like to encourage him to ask his GP for a referral to a specialist.
Can anyone suggest a way of approaching this issue without causing the employee further stress? Ideally we would like the employee to reduce the number of sick days they are taking and also to seek help and relieve of the condition.
And secondly, can anyone share a back to work questionnaire/form that we could introduce to all employees in the future?
