Does anyone have a good “staff satisfaction survey” questionnaire?

I want to accurately gather feedback from the organisation (55 staff) in a consistent manner to identify what’s working & what’s not. If anyone has used a decent tool in the past, I’d be very happy to know… I want to gather opinions on things like satisfaction with the management, quality of our product, etc. etc.
