Form Med 3 or Form Med 5

Can anyone tell me the difference between a Form Med 3 and Form Med 5 issued by a GP. I have been issued with both types for an employee we are disciplining due to persistent absence and non reporting of sickness absence.
If someone could tell me I would be grateful.
