How to encourage teamwork

I would be interested to hear from anyone who has successfully managaed to change the culture of an organisation to encourage teamwork at all levels.

This seems to be lacking at all levels, including the senior management team and HR has been tasked with the job of assisting them to achieve this!

I would be grateful for any help.
