We have a member of staff who is currently on long term sick leave for "Anxiety and Depression secondary to work related issues".
In November we requested GP details so we could get for a report on the medical condition with a view to assisting a return to work, the worker verbally agreed this but has not supplied the consent form.
The worker has been on sick leave since beginning of December having taken over 30 days sickness throughout the year. During January we requested consent to arrange an OH examination in order to gain an Independent medical report. The deadline to receive this was last Friday but no consent was received.
The worker has offered various excuses for their lack of cooperation, i.e communication difficulties through moving house (and not informing us), and our emails going into trash mail rather than their in box etc. The worker did email me last week to say they would complete the consent form and return it but it has not arrived.
I believe the next step is for us to arrange an investigation meeting and invite the worker to attend. We need to get this worker back in the workplace asap or find a replacement. Any advise on how to conduct the next steps would be very welcome.
