Paternity leave and pay form SC3

Hi,

I’m thinking of simplifying our paternity leave and pay request procedures, as our current process is a little convoluted.
My proposal would include merging our current leave request form with the official SC3, to create a single form process.
Do HMRC allow employers to create their own version of an SC3 provided it has the key information on it?
