I am currently gathering information on the best ways of compiling useful statistics on staff turnover. I would appreciate it if members could give me any ideas on how you operate this in your own organisations.

In particular:

What information is gathered by exit interviews / questionnaires, by whom and how useful it is to investigate and analyse data.

And how best to present such a management information package to a board of directors.
