I would like to introduce exit interviews, particularly as we took over another company September 2008 and this is our first leaver. I think it would be interesting to see if the changes to management has had any bearing on the reasons to leave. Is anyone willing to share their exit interview forms so I can assess what type of questions to ask and what can be of benefit.
