I am currently pulling together data for an absence project and wonder if anyone has produced a questionnaire for completion by Managers and or employees. If so I would be grateful if they could share they questions so I can use as a guide. I would also be interested in if the data received after circulation of questionnaire was useful and therefore a good source of primary data. Thanks
