Is there any such thing as an “employee retention questionnaire”?

By that I mean a questionnaire sent out in order to measure their willingness to remain with an employer. It could also be sent out to past employees to find out what might have persuaded them to stay with a previous employer.

Would anyone know how to design such a questionnaire as this?
