HR 'Service' Survey

I am looking to carry out a survey to gauge intenal customer opinion regarding the service they receive from the Human Resources Department.

Has anyone every carried out such a survey and if so, is anyone prepared to share a copy of the survey with me?

Otherwise, can anyone point me in the right direction as to where I might find a survey that I can adapt?
