On-line performance appraisal system – any recommendations?

We are an engineering company and are looking to introduce an on-line annual performance appraisal system.  We would like one that is configurable and ideally produce graphics – a spider or radial diagram would be perfect.

Any suggestions of systems you would recommend would be really helpful – and of course any to avoid.
