I am subject to disciplinary proceedings. I was interviewed and a transcript was made. I was provided with the record of interview four months later. I have a poor recollection of the interview and so do not see fit to sign it. I consider four months an unreasonable delay in providing the record.

1. Do I have a right to see the transcript?

2. Would it be reasonable to ask to be interviewed again at which time I can make a contemporaneous record?
